Well, if you're going to check out expansions, I personally recommend starting with Nightfall. It's the second-best of the campaigns in my opinion. Factions is great if you really enjoy PvP, but it's PvE aspect is the weakest of the three. If you want to get into Eye of the North, however, you can simply bypass those two campaigns (they are technically stand-alone titles, not expansions) and just jump right in. As long as you've reached Lion's Arch in Prophecies (the original) you can move into Eye of the North.
As for new mechanics, they've introduced two new things I think you'd like:
1.) Heroes - these little lovelies are NPCs like henchmen, but they're actually intelligent. You set their builds, you give them armor, you give them weapons. They do the rest. You can set them to various levels of combat activity, and the fun part is that they actually play builds well. They seem to have a built-in intuitive sense of how builds should work, and tend to play them in the most effective manner possible. It's very impressive.
2.) PvE Skills - these skills are awesome. Each of them has a different and unique effect, and are only usable in PvE. This is mostly because the effects they have would often make the PvP aspect of the game so unbalanced it isn't funny. For instance, one of the Ebon Vanguard skills allows you to summon an assassin henchman to fight against a selected target for a given span of time. And that's just one skill; there are tons of other PvE skills with various effects on the world around you, on NPCs, and on enemies. It's clever and innovative and something I haven't seen done before.

I will definitely have to check this out when I have some time (time, what is that?). I do not think I have gotten to the place you mentioned in prophecies. I would have to do that first.
It sounds like some great stuff. I have continually been impressed by their ability to keep a game afloat on the free to play model. I kept waiting to hear the announcement that they were tanking, but expansions just keep coming out.
